Fuller Life Counseling Partners offers targeted case management services to Medicaid eligible individuals who meet certain criteria. Case management services are ideal for those who have a severe mental illness, substance use disorder, or complex medical issues and need additional support and assistance to accessing resources in the community.
What can a targeted case manager do?

Assess the needed resources that can assist a client in being more successful in achieving some of their goals.

Collaborate with the team of providers working with an individual to ensure good communication among providers.

Facilitate meetings among treatment providers.

Connect an individual to wraparound services in the community and teach them how to access those services in the future.
